To obtain the childcare service voucher scheme, you must provide the following documents:
- the child’s identification number
- your income tax form issued by the Tax Administration. In the absence of your tax slip, the declarant must provide a certificate that he/she is not subject to the obligation to file a tax form
- the last three monthly household pay slips
Failing this, the operator will be obliged to apply the maximum rates foreseen.
